Care for All

The project proposal "Care for All" provides the creation of a Center for hourly service provision for social inclusion in the town of Lukovit. The staff will be a multidisciplinary team - socioconsultant/pedagogue, psychologist, medical assistant with medical education, home assistants and personal assistants. The center builds on a Department for social activities "Help at Home" and will be located in a wing of a municipal building. A repair of the premises is envisaged, aiming to create an individual boiler heating and suitable furnishing for disabled adults. Separate rooms will be established for social and psychological consultations and health corner for manipulations and rehabilitation exercises. The center will provide a set of innovative services for social inclusion in the community and at home - supporting and integrated cross-sectoral services. The services will be available to at least 78 users - people with disabilities and their families and people over 65 with limitations or inability to self-service, including from March 2016 the users of the scheme "New Alternatives". Rules and methodology for the activities of the Centre will be developed to ensure quality of services and multidisciplinary approach. The staff will undergo training – an introductory or supporting, and during the whole period of the project will receive psychological support, counseling and supervision to prevent professional burnout. All users will be assessed and will have social assessments and the results will be used to determine a complex of cross-cutting services that will be provided, including "home assistant", "personal assistant", “health assistant”, social inclusion, motivational and psychological support. The services will be provided both - in the center and at home - mobile services. The project will be managed by a team of municipal employees who will monitor and control the activities and will keep technical and financial documentation and reports.
