Support for independence and social integration of the elderly and people with disabilities in the municipality of Malko Tarnovo

The Application providing the implementation of a multidisciplinary approach to providing social and health services through the expansion the activities of the Unit for services in the home environment to the Home Social Patronage in the municipality of Malko Tarnovo, introducing a new kind of health service - health care and rehabilitation at home. Considering the specific needs of disadvantaged people in order to meet the complex needs of people with disabilities in the municipality of Malko Tarnovo will be used method of individualized services. In supporting work of the Unit for services in the home environment to the Home Social Patronage will be employs the staff of positions: "Personal assistant" and "Domestic worker". Depending on the possibilities and the specific needs of users will be employed 11 personal assistants and 7 domestic helper. Will be introduced integrated specialized health services, including medical care, counseling and rehabilitation for people with disabilities. For this purpose to the Unit will be employs the staff with employment contract, including - 1 manager - social worker, 1 nurse and 1 physiotherapist, will be bought mobile devices for ultrasound and magnetic rehabilitation at home. The project envisions the providing psychological support and consulting to users of the service, and consultation and supervision of staff. To ensure adequate, effective and responsive to the needs of people with disabilities social infrastructure and to improve the working conditions of the personnel providing the services will be carried out repair of the premises used by the Unit for services in the home environment to the Home Social patronage. After completion of the repair will be bought and delivered furniture and equipment to the premises required for the normal functioning of the Unit and providing quality health and social services.
