"Indipendent life at Lyaskovets Municipality - through integrated services for social inclusion"

Provision of integrated service for people with disabilities and elderly people that have difficulties to take care for themselves, through support to the Unit for services in home environment, created within the procedure "Assistance at home" and further development through the establishment of a Unit, which will provide long-term care including social and health services at home. Improvement and further development of the social services - "Home assistant", "Social assistant" and "Personal assistant" as a form of social services in the community depending on the specific needs of the disadvantaged people, based on their individual assessments. The project proposal will be implemented through the following activities: Activity 1: Organisazional and management activities for effective management of the project - project team; Activity 2: Establishment of a Unit for hourly delivery of services for social inclusion at home (Unit) through the improvement of the material base - renovation of premises, furniture and equipment of one office with 2 work places, 1 room for group activities and 1 sanitary premise. Activity 3: Formation of a team of specialists which will work at the Unit an will provide integrated services follwing clear procedures which will apply multidisciplinary approach in the complex of social services. Activity 4: Selection and contracting staff for the delivery of hourly social services at home; Activity 5: Selection of users for hourly services for social inclusion within the Unit; Activity 6: Provision of hourly social services for social inclusion; Activity 7: Training and supervision of the experts and staff of the Unit for hourly services for social inclusion; Activity 8: Popularization of the project.
