"Home Care for Independent Life" Center in the municipality of Kresna

The project upgrades the services in home setting from previous projects and programmes by applying flexible mechanisms for combined hourly usage of services, oriented to the personal needs of the user. We plan and control qualitative services for personal help and health care, help in communal-household activities and service for social inclusion and support. Through consultation, motivational and psychological support, performance of individual plans and developing programmes, through providing integrated services from multidisciplinary team we go towards providing long-term care and creating opportunities for disadvantaged people to control their environment and live by themselves. The representatives from both target groups get access to long-term social-health care by providing integrated services according to the specific need of disadvantaged people. The project includes people with serious social and health problems from both target groups - people with permanent damage and their families and people over 65 with restrictions or inability to self-service. The project creates new work places, as 51 people are included again in the labor market; the professional skills of the local human resources are increased through training and supervisions.
