"Dignified Life for All"

The project envisages the creation of a new Center for hourly service provision for social inclusion in the community and at home. Support for social inclusion in the community or at home will help to overcome the lack of access to basic social and health services and achieve independence through integration. The project aims at improving the quality of life and access to services for inclusion in the response of complete needs, including health, people with disabilities and people over the age of 65, unable to self in order to overcome the consequences of social exclusion and poverty. The project implementation will complement and build implemented by the municipality and other social projects will provide a wide range and scope of activities towards providing integrated social and health services. Seniors and persons with disabilities will have a choice of services for their specific needs. The center will provide these integrated social services- "Personal Assistant", "Social Assistant", "Home assistant", "Health Assistant". The project envisages the inclusion of 100 representatives of the two target groups. In addition to basic services will be provided and motivational, psychological and other support. The implementation of the activities at the Centre will be implemented by: Manger-1; Psychologist-1; Caseworker-2; Doctor /medical auxiliary- 1; Person for occupational therapy-1; Physiotherapist / rehabilitation therapist-1; total personnel: Accountant - 1; Driver / when purchasing a car / - 1; Technical Assistant / - 1; Personal Assistant - 5 +40; Home Assistant - 30; Social Assistant - 5; Healthcare assistant -5. The team of the Center will be provided - basic and advanced training and supervision. Their activity will be supported by the development of rules and methodology for offering social services provided. To provide access for people with disabilities will be delivered specialized vehicle.
