Provision of social services procedure "Independent Living" by the Municipality of Koprivshtitsa

The project foresees providing access to integrated services for social inclusion in the community or at home, including supporting integrated and intersectoral services to the specific needs of people disadvantaged on the basis of an individual assessment by social support and expansion of the existing unit for hourly service provision created under the project "Assistance in the home." In the unit will be presented in an innovative way to needy persons living in the municipality of Koprivshtitsa three types of hourly services at home - for personal assistance, social support and assistance with public utility activities, as well as additional health and social services environment other than your home, namely health services, rehabilitation services, motivational and / or psychological services to needy persons for a period of 18 months. They will be appointed under labor contracts 24 persons who will provide services for 38 users. / This includes staff and users of the project "New Alternatives", which will be included after 01.03.2016god /. This project proposal of Koprivshtitsa municipality meets the general and specific objectives of the scheme "Independent Living" in the following way: official representatives of the target group will puluchat access to long-term health and social care, incl. and access to employment, according to the specific needs of people disadvantaged on the basis of individual needs assessment; - Establishes a decentralized model of sustainable provision of social services and to ensure conditions for a decent life for persons who are or are at risk of social exclusion; -predostavya possibilities for the return of the real market of people who care for loved ones with disabilities.
