" Service Center in a home environment - an integrated model of social inclusion for independent living in small rular settlements type in municipality Tundzha "

The essense and the task of the project is to weed out specific complex model for an integrated approach pointed to provide,complete, effective and efficient medical and social care for elderly people with chronic diseases and people with disabilities at home environment in the small rural settlements type by creating the Centre for services in home environment. Within 18 months will be offered hourly service from trained staff, using a especially developed for this purpose Methodology, which will be adupted to the individual needs of consumers - adults over 65 and people with disabilities and their families. Project implementation support and builds on the activity of "Unit services at home environment," built under the scheme "Help at Home", financed by the Operational Programme”Human Resources Development 2007-2013”, for providing hourly service for social integration in the community and gives an opportunity to create a new structural model for alternative health - social services in the home environment to encourage and support the process of social inclusion in the 40 rural settlements type in the composition of the municipality "Tundzha". Project implementation will provide employment for at least 35 unemployed people of working age, and the other will encourage and support the process of deinstitutionalization by creating the possibility for the inclusion at least 90 people - elderly people over in '65 and persons with a disability to get complex care at home. Providing integrated health social services will create conditions for an independent and dignified life for the elderly and people with disabilities by providig quality, accessible and sustainable services for long-term care according to their individual needs in achieving a better balance between quality, effectiveness and efficiency services, generating high added value and the experience of the beneficiary in the delivery of such services financed by Operational Programme”HRD 2007-2013”
