"A second chance for care"

Current project responds to the need to restructure the existing unit for service in a home environment in the Center for Social and Health Services, by expanding the proposed hourly services the in a home environment and provide integrated access to health, social and other services according to the specific needs of people in disadvantaged position based on their individual assessments. Through this innovative way will allow to output the these services from close family range of the disabled and provide real opportunities for family members involved in the care of needy relatives for work. Sredets municipality provides for free use the second floor of a building municipal property, located in Sredets, street "Brothers Miladinovi". The building is renovated furnished. To provide hourly services within people will be hired to serve people in need. The CSHS team will be composed of 28 people. The providing of services will be based on identified needs, problems and social assessment of individual representatives of the target group - children and persons with permanent disabilities in need of permanent care and adults with limitations. We anticipate the project to serve 63 people. Social assessments will be carried out by a governor from the Center. Services - personal assistant and a home nurse will be provided in the homes of consumers. The set of social, health, psychological, rehabilitation services will be provided on an hourly basis, as in the Centre and in the homes of consumers. For this purpose project we have planned to buy a specialized car with a ramp for disabled people, which can be transported customers from their homes to the center and back. For quality service provision impossible by consumers, experts predict mobile work.
