Independent living for the citizens of Sarnitsa

Establishment of a Unit for part-time social inclusion services in the community or at home at the territory of Municipality of Sarnitsa. The project implementation will provide integrated services for people with inability for self-service and for people with disabilities, as it will be combined complex steps towards providing long-term care, including health and social services in the community or at home environment Target group: 1. People with disabilities and their families 2. People over 65 years with limit or impossibility in self-service The project will be implemented through actions and measures that will be implemented in partnership with social service provider FOUNDATION "ENVIRONMENT, HEALTH SOCIETY SCIENCE" - OZON, until the Additional agreement 2 enters into force, with which the partner's involvement in the project activities is ended, to achieve the planned results in the project proposal, as follows: 1. Project management and organization. Selection and matching of consumers and providers of social services. 2. Providing part-time social services in the community and at home environment. Providing access to integrated services, according to the specific needs of ‏ disadvantaged people on the basis of individual social valuation by supporting the activities of the Unit for hourly service provision. 3. Staff training 4. Providing the psychological support and/or consulting and/or supervision of the staff, providing social services. 5. Motivational and/or psychological or other kind of support for service consumers according to their individual needs. 6. Information and publicity As a result of the executing the activities will be improved the quality of life of elderly and disabled people by creating conditions and support for the effective prosecution of their right to an independent living and social inclusion, with respect for their rights, to comply with their specific abilities and needs.
