Providing integrated social and health services to persons who are dependent on a care in Devin Municipality

The project proposal plans to provide integrated social and health services by dint of Service Centre at home in Devin town, which has been created within the grant scheme "Help at Home". The acceptance of applications for the project is expected to be accomplished in two stages: o the first stage - at the start of the project o the second stage - the last month of the period of providing the service "Personal Assistant" according to the project "New opportunities for care", the grant scheme "New Alternatives". It is envisaged that individual assessment of needs / of two stages / will be done on the applicants for the services within the project. An organization –contractor will be selected to carry it out. It‘s planned that two introductory courses will be conducted / one for each stage of the acceptance of documents / and a supervision. The following personnel of the Centre will be hired for the providing of the services: 1 Manager; 1 nurse; 1 psychologist; a total of 75 people to provide services at home - 25 domestic and 50 personal assistants. The Services will be provided in all settlements in the municipality of Devin. The purpose of this project proposal is to improve the quality of life and access to health and social services to persons with disabilities and persons over 65 years with inability for self-service. A set of social and health services will be provided according to the needs of the applicant for service users within the project and their individual assessment. On one hand, employment will be provided to unemployed people of working age, and on the other hand- enable persons with disabilities will be able to choose the persons who will provide their services at home and in the community. The project implementation will enable service to the users to lead an independent lifestyle and will provide a support for the effective exercise of their right of independent living and social inclusion.
