Hope for Independent Life A system of social and health care by creating a center for services at home Nedelino

The Municipality of Nedelino is a candidate for this project proposal. The Municipality is situated in the central south part of the East Rhodopes, and its territory is 102.3 square km with an altitude of 400 - 1200 meters and includes Nedelino town and 15 settlements. With the realization of this project, a center for hourly care for social integration will be created with the goal of guaranteeing an integrated approach in order to provide social and health care to those who most need it. The care will be provided according to individual social assessment of each beneficiary of the group. The purpose is to create a sustainable model to facilitate productive lifestyles and provide care at home for those who have disabilities which impact their ability to care for themselves and therefore also at risk of social isolation. By implementing this project, we would be enabled to provide various types of hourly care at home including personal care, social support, communal and household activities and health care. In the target group of the project are: - Disabled people and their families - People older than 65 who are disabled and dependent on others for self-care In Nedelino, being a typical Municipality of villages, four negative trends dominate in comparison with larger towns: -people do not live as long -they suffer from more illnesses -their access to health care is limited The complex demographics and at risk residents based on limited health care access in Nedelino Municipality suggests that it would benefit greatly from this project. The intent to help provide the resources these disabled residents require in order to lead more productive and possible independent lifestyles offers us hope. This striving for independent life is connected with the basic human rights. Independent life means to have the opportunity to live like other people. The term for providing a system of social and health care at home is 24 months.
