Center for Integrated Services, Dimitrovgrad

The main priority of the project proposal "Center for Integrated Services - Dimitrovgrad" is to improve the quality of life of people with disabilities and people over 65 years of age who are incapable of self-service, by providing an access to basic health care services, social services, etc. For the above mentioned purpose it is foreseen a Centre for hourly provided services for social inclusion to be established in the community or in a home friendly environment, in which a team of professionals will operate - social worker, a health care specialist - nurse and a psychologist. All of them, together with a consultant, will prepare an individual social assessment of the candidates eligible to become consumers. Based on this assessment social services, health care services and other services to the approved candidate will be provided, after signing an Agreement for providing social services. Staff formed by personal assistants, social assistants and domestic servant will be hired and trained to be able to provide the social services. Health care services will be provided respectively by a nurse hired as a staff member in the Center. A psychologist will perform psychological support to the staff members which will provide social and health care services to the clients according to their individual needs. The leading principle in the project implementation will focus on the specific needs and requirements of each consumer and an integrated access to health care services, social services and other services to be provided in order to meet the aforementioned needs and requirements.
