Hand in hand

Hand in hand project is aimed to provide access to social inclusion services facing the complex needs, including health issues, of disabled people and people over 65 years incapable for self-servicing from Loznitsa Municipality through establishing of Center for hourly social inclusion or home services delivering. To this end we are going to provide necessary equipment, furnishing and vehicle for specialized transport for services users, we are going to select and train a team for managing and delivering these services, including 30 personal assistants and home care assistants, we are going to provide rules for working and service delivering by the Center, we are going to select grounding the analyzed needs, the Center’s services users, we are going to deliver to 30 users the necessary complex services, including at home, for social inclusion for 18 months. We are going to deliver phsihological assistance, consultations and supervision for Center’s team as well - motivation and phsihological assistance to Center’s services users. Project target group is: 25 disabled people and 5 people over 65 years incapable for self-servicing from Loznitsa Municipality. The key project result is to be improved access to hourly delivered services for social inclusion or in home for 25 disabled people and 5 people over 65 years old - incapable or with limited capabilities for self-servicing - from Loznitsa Municipality. This access is to be ensured through Services Delivering Rules including the procedure for users’ needs assessment, selection and providing the delivering, motivated and experienced services delivering team, contemporary delivering facility and provided specialized transport. The project is to be run for 24 months by designated on this purpose experienced and motivated management team: leader, coordinator and accountant on the territory of Loznitsa Municipality.
