Reducing poverty and promoting social inclusion.

Support will be aimed at facilitating access to health services and to the development of social services in the community to support the social inclusion of people with disabilities and facilitate their access to employment. As a result of activities will improve the quality of life of elderly and disabled people by creating conditions and support for the effective exercise of their right to independent living and social inclusion, with respect for their rights, to comply with their specific abilities and needs. The implementation of the transaction will provide integrated services for people with inability for self and for people with disabilities, will combine complex steps towards providing long-term care, incl. and social and health services in the community or at home. Providing employment to unemployed people of working age and enable people with disabilities to choose the desired and necessary assistant and will reassure environmental control. Each person with a disability, according to their needs, can use various services as required and the type of aid. It is possible that each of the service providers responsible for different types of activities. This is decided by the user. The aim is to provide representatives of the target group access to long-term health and social care, incl. and access to employment, according to the specific needs of people disadvantaged on the basis of an individual assessment of needs.
