Service Center for Independent Life in Vazdrazhdane

The main objective of the project proposal is to improve the quality of life and access to services for the people of the target group and help them to overcome the consequences of social exclusion and poverty by providing a complex health and social services. Specific objectives: to create a center, which would provides social and health services in order to fulfill the specific needs of each user of the Centre; to build and train a multidisciplinary team of the Centre to provide services to the target group in their homes; to promote best practices and results. Target group: People with disabilities and their families; People over 65, who need assistance in order to achieve or maintain independence: 85 persons from the target group Activities: Organization and project management ; Information and publicity; Creation of a Centre ; Staff training; Purchase of specialized vehicles for transporting users of the Centre; Assessing the needs of the people from the target group. Set up a center, which will provides hourly complex services in the community or at home in order to help people of the target group to overcome the consequences of social exclusion and poverty. Expected results : organized regular meetings of the management team - 22pcs.; a selection procedure for external contractors; final meeting ; made two publications in the media; publish information of the project on the website ; made and placed information banner - 1 ; recruitment staff who would manage and provide necessary services for good performance of the centre; conducted induction training of newly hired people; developed a methodology for the Centre; purchased a specialized vehicle; An assessment of the needs of 85 persons of the target group report; Set up a Center, which woud provided social and health services of 85 representatives of the target group.
