Centre for hourly services for social inclusion in the community and home environment in the municipality of Valchedram

The project duration is 21 m.The team of organization and management will organize whole activities,will monitor a time schedule, achieve performance indicators, proper expenditure of the funds, through permanent direct monitoring.This team will make the selection of external experts for services, personnel recruitment and consumers of the Center,will monitor the implementation of supplies, repairs,furniture and equipment,specialized transportation,informational materials,according to developed and approved schedule.For functioning of the Centre the Municipality provides office, which will be repaired properly furnished and equipped with funds from the budget of a project.There are 2 rooms for rehabilitation,psychological support,medical care, social support.For providing access to services at the Center will be purchased specialized transport for the users.At the beginning the Center will serve persons with severe health limitations or inability to self- service not covered by other programs and projects.The people who are needed,will submit applications for use of integrated services personal, social, home assistant,psychological support,social support,health care at the current address.All applicants will be assessed and the team of the municipality will make a final selection,according the selection procedure for criteria.In March will be made again admission procedure of applications and evaluation of users of Project New Opportunities for care.About the staff selection will be designed a procedure and criteria.The integrated services will be providing of hired 54 employees,4 specialists and 1 driver.An external expert will develop a methodology for services providing.The personnel and specialized team will pass introductory and two advanced trainings, which will ensure the quality and regular services.The directly working with target groups team will receive support from external expert team periodically.Provided are activities for information and publicity.
