New Horizons

The aim of this Project Proposal is to establish a Centre for per hour delivery of social inclusion services in the city of Targovishte, that will employ a staff composed as a multidisciplinary team including social workers, a psychologist, a rehabilitator and a supervisor, a health care worker, domestic assistants and social and personal assistants as well. Under this Project certain routine repairs, furnishing and fitting up of the premises are envisioned to make them comfortable for disabled people. Special rooms will be equipped for social assistants and for psychological consulting. A health care sector for medical treatment and rehabilitation. will also be set up. The Centre is designed to deliver a set of innovative services – both supporting and integrated cross-sectoral services aimed at social inclusion within the community and in the domestic environment. Access to those services will be provided to a minimum of 90 users disabled people and their families, as well as to the people aged 65 and above with limited or lost ability to care for themselves Rules/methodology for the Centre’s activities will be developed to ensure quality of the service delivery and the multidisciplinary approach. The staff will receive both introductory and supporting training, as well as psychological support, consulting and supervision throughout the Project duration to prevent job burnout. Social evaluations will be performed on all users to define their individual needs as regards particular services they may require: domestic assistance, personal, social or health care assistance, social inclusion, motivational and psychological support. Services shall be provided either at the Centre premises, or at the users’ homes-mobile services The Project shall be managed by a team composed of municipal servants, who shall monitor and supervise the project activities, prepare technical and financial documentation and reports
