This project will give Lyulin an opportunity to provide hourly integrated services to citizens with impairments as well as to such over the age of 65 years with impairments and inability for self-care.The realization of the project will lead to improvement of the quality of life of the elderly and the people with disabilities through creation of conditions and support for effective application of their rights to independent living and social inclusion, taking into consideration their abilities and specific needs.Providing integrated services in home environment will support on one hand the users, while on the other hand it will support the members of their families who are primarily engaged in care for their disabled relatives thus reducing the risk of dependence on institutional care as well as giving the healthy family members a chance to engage in real employment.The hourly provision of integrated home care gives an opportunity to increase and diversify the services that meet the needs of the target group. Rules and requirements will be developed for the social care set offered in Lyulin in order to guarantee the said set`s quality and multidisciplinary approach. In order to ensure the application of the integrated approach the team that will provide hourly integrated services in home environment will include specialists with different areas of expertise –social, medical as well as other qualifications. he support in home environment will be provided by personal caregivers, medical assistants and home helpers. The team will provide motivational and psychological support according to the individual need of the service users. Through the purchase of a specialized automobile so adapted as to meet the needs of physically impaired, we will ensure that they (elderly and disabled alike) have their transportation needs met. From April 2016, after a new needs evaluation we will include as users the people who are currently using the `New Alternatives` service.
