Improving the quality of life and the access to services for social inclusion in Sofia municipality - Region "Novi Iskur""

This project proposal will allow in Novi Iskar region to provide hourly integrated services to people with disabilities and elderly people over 65 years with limited or unable to services themselves. The implementation of the project will improve the quality of life of elderly and disabled people by creating conditions and support for the effective exercise of their right to independent living and social inclusion, respecting their rights and complying with their specific abilities and needs. Providing integrated services in their home environment will support on one hand the consumers and on the other hand the dependent members of their families, reducing the risk of dependence on institutional care and a chance to include members of their families in a real employment. Hourly provision of integrated care in a home environment allows an increase and diversification of services and meeting the needs of the target group. There will be developed rules and requirements for the complex of services that will be provided in Novi Iskar region to ensure their quality and multidisciplinary approach. The team will provide hourly integrated services in the home environment will include experts who have a different profile of competence - social, health education and other forms of vocational training to ensure an integrated approach. Support at home will be provided by personal assistants, medical assistant, domestic helpers. The purchase of an adapted van will provide much-needed specialized transportation for the elderly and people with disability. Since April 2016, after making new needs assessments, the activities of this project will include consumers using services on procedure “New opportunities for support”.
