Opening of the Centre for hourly service provision for social inclusion in the community or at home in the municipality of Dupnitsa

Overall objective: To improve overall well-being and access to services for social inclusion of people with disabilities and people over 65 in the inability for self service and / or their families by providing innovative integrated services for social inclusion. SPECIFIC objectives Opening Center for hourly service provision for social inclusion in the community or at home in the municipality of Dupnitsa risk prevention of institutionalization and reduce the effects of social exclusion and poverty of people with disabilities and people over 65 unable to self-service. Approbation of innovative remote monitoring service "Telecare" to prevent existing health risks in the home environment for people with disabilities and people over 65 unable to self-service. Improvement of the existing model social services at home in the municipality of Dupnitsa and upgrade results of the project procedure "New Alternatives". The project proposal is aimed at people with disabilities and their families and people over 65 with limited or unable to self-service. According to the Guidelines for Applicants are expected to be included and persons from the target group who use services under procedure "New Alternatives" after completion of the project and after being reassessed their needs. The project will cover 120 representatives of the target group - direct beneficiaries.
