The project aims to improve access to basic social and health services and achieve independence and social integration of children, adults and people with disabilities. Project activities will contribute to expand the opportunities for improving the quality of life of vulnerable groups and/or their families provided in the previous programming period, by promoting equal opportunities for vulnerable groups and expanding the volume, diversity and targeting of social services provided in the community. The implementation of the action will provide integrated services for people with inability for self service and for people with disabilities, will combine complex steps towards providing long-term care, incl. social and health services in the community or at home.
