The analysis of the provision of social services in the community and / or at home showed that the service has a strong social impact to improve the quality of life of people with severe disabilities, relieve their families and access to employment. This project proposal aims to respond to the increased need of people disadvantaged by social services in the community or at home. Overcoming the social isolation of families in which there are people with disabilities and people living alone over 65. That are in partial or complete inability to self alone at home to organize independently their lifestyle and daily activities. Priority objective of the operation is to offer affordable, high quality and efficient social services that meet the needs of consumers with stronger involvement of all stakeholders, provide social inclusion, living in a supportive community and full realization. For effective realization of the objective need arises Center for hourly delivery of comprehensive integrated services for social inclusion in the community and / or at home like: medical / healthcare, rehabilitation; therapeutic activities for development; psychological support, social advice for users and their relatives. Of disadvantaged people, according to their needs, will provide an opportunity to choose and use different types of services and assistance. Along with the provision of social service in the community or at home, providing access to employment for unemployed people of working age and ability of vulnerable groups to choose their own desires and trusted assistant, which is especially important and place.
