Center for hourly and maintenance of the services for social inclusion in the community or at home, including supporting cross-sectoral and integrated services

This project proposal will allow in Area "Kremikovtzi" to provide hourly integrated services to persons with disabilities and elderly people over 65 years with limited or unable to self-service. The implementation of the project will improve the quality of life of elderly and disabled people by creating conditions and support for the effective exercise of their right to independent living and social inclusion, respecting their rights and based on their specific needs and opportunities. Providing integrated services at home environment will support on the one hand the consumers and on the other dependent members of their families by reducing the risk of dependence on institutional care and a chance to involve members of their families in a real employment. Hourly provision of integrated care at home environment, allows for an increase and diversification of services and meeting the needs of the target group. It will be developed the rules and requirements for the complex of services that will be provided in the region "Kremikovtzi" to ensure their quality and multidisciplinary approach. The team to provide hourly integrated services at home environment will include experts who have a different profile of competence - social, health education and other forms of vocational training, to ensure an integrated approach. Support at home will be provided by personal assistants, medical assistant, domestic helpers. The team will provide motivational and psychological support, according to the individual needs of users. With the purchase of an adapted van, will provide much-needed specialized transportation for the elderly and persons with disability. Since April 2016, after making new needs assessments we will include the activities of this project consumers using services on the procedure "New Alternatives".
