"Creation of a Centre for hourly service provision for social inclusion in the community or at home in the municipality of Anton to ensure independent living for people unable to self"

The project proposal improves quality of life and access to services for social inclusion of people with disabilities and people over '65 unable to self through social health services. The project aims at providing the service "Personal Assistant" and social assistant in a home environment and provide comprehensive services to the target group in and outside the home environment created Center for part-time service delivery. Planned activities build on the information provided to date service "Personal Assistant" and reveal a great opportunity for people with disabilities and people over 65, unable to self through social health services in the newly opened Center for providing complex services with the inclusion of a social worker and psychologist . For the realization of the project proposal and a target following activities are planned. 1. Activity in organization and project management; 2. Activity of information and publicity3. Provide access to integrated services, according to the specific needs of people disadvantaged on the basis of an individual assessment by new social center for hourly service provision for social inclusion in the community or at home, including supporting cross-sectoral and integrated services; 4. Training of personnel services; 5. Provision of psychological support, counseling and supervision of personnel providing social services; 6. Motivation, psychological and social support for service users according to their individual needs. 7. Development of rules for the activities of the Centre for hourly service provision for social inclusion in the community. 8. Development of social assessment. 9. Activities to perform repair of municipal buildings to create the Center for hourly service provision for social inclusion in the community. 10. Furnishing Center hourly service provision
