SUPPORT ME

The aim of the project proposal is to create sustainable model for provision of dignified life through providing access to long-term health and social care, incl. employment for people in partial/complete impossibility to self-service and at risk of social exclusion. Following the Municipality main strategy’s vision and objectives, the project activities will support the „Home environment services” unit /part of Home social patronage/, created by the procedure "Help at Home". The Unit will provide integrated hourly services-separately/within a group, including following activities: personal care; social, medical and psychological support and social inclusion; utilities. The project team will be responsible for all planned activities, to realize operative monitoring, to prepare reports, to consult the beneficiaries and target groups. It will start with information campaign about project objectives and activities, the circumstance to make use proposed services and terms of employment of social workers. The Unit staff will organize all the activities for the preparation and delivery of integrated services according needs of elderly and disabled people - unable to lead independent lifestyle, according to their personal assessment. Domestic health-officers will perform different separately/ in a group personal assistance and communal activities according to the individual’s needs and desires. Social assistants will provide support and inclusion activities. Personal assistants will care about their relatives with disabilities – this will ensure their inclusion in the real labor market. A nurse and a psychologist will ensure complementary care to beneficiaries. The implementation of project activities will give chance to people over 55 and people with disabilities to choose a suitable service and at same time to assist them to take decision about required support. All untrained hired persons will pass special training and supervision to provide better social services.
