Independent life for citizens in the Municipality Hayredin

The overall objective of the project is to contribute to improving the quality of life and the consequences of social exclusion and poverty of people with disabilities and people over 65 years of Hayredin Municipality by improving access to integrated services for social inclusion. To achieve this objective we are planning construction and equipment of Unit for hourly service provision for social inclusion in the community and in the home environment and the provision of integrated services to representatives of the target groups. To expand the volume, diversity and targeting of services, will be built a multidisciplinary team of specialists - social worker, a health worker and a psychologist. In the Unit disabled people will use social, administrative and medical services, they will have the opportunity for social contacts, access to current information. In their homes will be provided home services assistant and personal assistant. Users involved in the project will use motivational and psychological support from a qualified specialist. It will be carried out an individual assessment of the needs of each user of the Unit in order to ensure that each user will receive a service or range of services that are needed. The project will be appointed 18 home assistans and 33 personal assistants who will take care of 69 people with disabilities and people over 65 who can not serve themselves.
