Creating equal opportunities for independent living

This project proposal will allow "Triaditza" District of Sofia Municipality to provide users (persons with disabilities and elderly people over 65 years ), with integrated social services for social inclusion. Through the provision of integrated social services on hourly basis, District "Triaditza" will contribute for creation of equal opportunities for independent living of elderly people and disabled persons. The implementation of the project will improve elderly’s and disabled people’s quality of life by creating conditions and support for the effective fulfillment of their right to independent living and social inclusion, respecting their rights and complying with their specific abilities and needs. Providing integrated services at home environment will provide users/consumers with support - on the one hand, and on the other hand – it will support the dependent members of their families, by reducing the risk of dependence on institutional care and by creating a chance for involving family members in a real employment. For implementation of the project, "Triaditza" District will provide a room where will operate virtual "Center for Independent Living". A methodology, internal rules and requirements will be developed in order to ensure the quality of the social service provided in the Center, as well as the implementation of a multidisciplinary approach to be guaranteed. The Centre's team will include experts with various profiles of competence to ensure an integrated approach - social, medical, health, teacher training, and other forms of vocational training. Personal assistants, home helpers as well as social and health care assistants will provide users with support at home. The users will be given motivational and psychological support, according to their individual needs. In addition, the elderly and disabled users will be provided with specialized transport service "door to door", and for this reason, an adapted van will be purchased.
