CHANCE AT HOME

Improvement of the access to basic social and health services and achievement of independence and social cohesion for disabled or elderly people above 65 years of age, unable to care for themselves is the aim of the project proposal. The main project activity is the creation of new Center for hourly provision of services for social cohesion in the community or in house through which access will be provided to integrated services based on the individual social needs and according the specific necessities of the disabled. The implementation of the project will provide to the beneficiaries access to long-term social and health care in community or in house, including access to employment. Detailed rules and requirements for the activities provided by the Center will be elaborated in order to guarantee the multi- section approach and quality of the provided services. The project team will be comprised by experts with different skills. The provision of the services will be based on individual needs and problems of the beneficiaries and the individual estimation of each case. The implementation of the project activities will contribute for: - Return to real employment for people who at present care for their disabled relatives; - Provision of trainings, psychological support, consultations and supervision of the staff for the provision of the social services; - Provision of motivation, psychological or other support according individual necessities of the beneficiaries. The needs and necessities of every individual person will be the main focus in the selection and implementation of services. The implementation of the project will further contribute for extension of the possibilities to improve the quality of life of disabled people and their relatives, which were provided during the previous Program period through provision of complex services for long-term care in community or in house.
