"Complex care in a home environment on the territory of Djebel Municipality by LAG Ardino - Djebel"

The project proposal is aimed at providing a complex of services in a home environment for people with disabilities and adults over 65 years of age, unable to self-service ie. persons with difficulties in daily self-service on the territory of the municipality of Dzhebel. Its implementation will contribute to improving access to integrated services for the needy in order to overcome social exclusion and poverty, while building on the social services offered in the municipality. The integrated services will be implemented by the already established "Social Services Center - Djebel" through hourly provision of social inclusion services in the community or in the home environment, namely social and health. The center will employ at least 15 individuals hired for personal assistants, social assistants, home assistants, and a medical professional. Activities will be implemented over a period of 6 months and will cover at least 52 users. This will enable the users to choose the services they want, organize their distribution and their length over time, to define a caregiver to care for them, according to their specific needs. In addition, it will ensure the employment of persons of working age. Improving the quality of the service complex and the achievement of good practices will take place through the included activities for motivation or other support for consumers; counseling staff working in the center. As a result of the implemented activities, the quality of life of the representatives of the target groups will be improved by creating conditions and support for the effective exercise of their right to independent living and social inclusion, respecting their personal choices tailored to their capacities and individual needs. Project activities are organized in a way that ensures optimal use of human and financial resources.
