Patronage care for elderly people and people with disabilities - component 1

The aim of the project is to improve the quality of life and social inclusion opportunities for people with disabilities and the elderly by providing a network of home environment services and building appropriate physical and staff capacities. The specific objective of the project is to develop a model of a patronage service for elderly people and people with disabilities (including people with chronic illnesses and lasting disabilities) so as to provide a time-consuming integrated health and social care service in clients' homes. This model will structure and standardize the provision of the Patronage Care service that will be provided by the municipalities. As part of the project, it is planned to develop a specific training program for medical professionals and social workers who will provide patronage care, a patronage which program/methodology which will provide unified guidance to service providers, and a quality standards for the service. As well as the elaborated documents will be used as a resource-tracking and monitoring tool in the monitoring and monitoring of the Patronage Care service. The implementation of these activities will contribute to providing a new type of integrated health and social care services that are of the necessary quality and accessibility for long-term care in the community and will help the process of deinstitutionalization of the care of the elderly and people with disabilities. The project will be implemented on the territory of the country with a duration of 30 months and a total budget of 200 000 BGN.
