"New long-term care for adults and people with disabilities"

The project is part of the overall process of creating an independent living environment for people with mental disorders and mental retardation by providing quality and affordable cross-sectoral services of a resident type and tailored to the individual needs of individuals. It is implemented in accordance with the measures envisaged in the Action Plan 2018-2021 for the implementation of the National Long-term Care Strategy. Preparation for provision of services in the community will be ensured, through the establishment of appropriate infrastructure with funds under Operational Program "Regions in Growth". A Methodology will be developed to make individual needs assessments and support plans for people with disabilities; Methodology for closing down the homes for people with disabilities and for reforming the homes for the elderly. 22 assessment teams will be set up to carry out individual needs assessments and support plans for over 700 individuals from 10 specialized institutions designated for closure and over 200 persons residing in state psychiatric hospitals for social purposes for a long time. Training materials will be developed to train social and healthcare professionals providing social and integrated health and social services. The assessment teams and staff of the institutions designated for closure will receive support through supervision. Plans to close down the 10 specialized institutions will be developed. An analysis of the remaining 151 institutions for people with disabilities and the elderly will be carried out, which will be the basis for the development of the second Action Plan for the implementation of the National Long-Term Care Strategy. Active work will be done with the users of the institutions to prepare for their removal and transfer to the new services. A Communication Strategy will be implemented, whose main goal is to change the attitudes of society towards people with mental disorders and mental retardation.
