The implementation of specialized home care in Dryanovo municipality aims improving the access to health and social services for people with disabilities, their families and elderly people over 65, with limits or inability to self-service, according to their complex needs. As a result, it will be possible to achieve the independence and social integration of the vulnerable groups of people. The integrated method in the provision of hourly services, based on the individual needs of personal assistance, support for social inclusion and health services, will be a motive for higher quality and a decent life for consumers. The project proposal is oriented towards the users of integrated health and social services. There is an accent on the specific and individual needs of every user, on the basis of which will be formed the package of complex services. The “Home Assistance” project will enhance the integrated services, provided in the municipality, by completing care at home environment with specialized health services. The realization of the project will facilitate access to integrated services through their mobility and complexity in providing them. Giving a job of unemployed people is of particular importance for their inclusion in the social economy. The main project activities include providing access to integrated health and social services in the home environment; ensuring publicity and information; a social evaluation of the participants needs; professional training for the employee; supervision and psychological counseling; psychological support for service users; organization and management of the project.
