The project "Help old people/seniors and persons with disabilities through patronage care in the municipality of Karlovo" is aimed entirely at old people over 65 with limited or inability for self-care, as well as to people with disabilities and their families, people who have no relatives, for whom there is a social need for hourly services in their usual environment. In addition to home care there will be developed community-based services aimed at preventing loneliness and isolation. These services will also be adapted to the basic needs of the old people to help them stay independent and stay the longest possible time in their homes. People with disabilities will be provided with specialized medical care in a home environment, which will help build and validate a model for patronage care and the provision of hourly mobile integrated health and social services.
