"Patronage care for the elderly and people with disabilities " - Haskovo

This project proposal is aimed at providing quality personalized mobile integrated health and social services in a home environment for elderly people over 65 with limited or inability to self-service and people with disabilities and their families on the territory of Haskovo Municipality. The outlined services will be provided in an innovative way with intention to provide personal health, social and psychological help and support. This way they will contribute to the improvement of the accessibility to new integrated services for people in need of this and to overcome social isolation, while simultaneously is achieved building up of the already available social services in the community. A main accent is adequate provision of quality basic health and social services and psychological support at home for people living in remote settlements where access to these services is very limited or lacking. The project proposal foresees to include 177 participants in the borders of the Municipality of Haskovo for a period of 12 months. The main objective is the providing of a dignified and independent life in the context of normal life. As a result of the accomplished activities, the quality of life of the target groups will be improved by creating conditions and support in order to ensure independent life and social integration regarding their personal choice compliant with their own capabilities and necessities. The project activities are aimed at building of sustainable model for qualitative provision of patronage care on the territory of the Municipality of Haskovo and are organized in a way that ensures the optimal use of human and financial resources.
