Patronage care of elderly and disabled people in Parvomay municipality

The aim of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in the Municipality of Parvomay by providing a network of services in the home environment and building adequate material and personnel capacity for their provision by introducing a model for patronage care for elderly people and people with disabilities, incl. with chronic illnesses and lasting disabilities to provide hourly mobile integrated health and social services in their homes. The implementation of the project will be carried out by assignment of the patronage service to the Home Services Service Unit to the Domestic Social Patronage - Parvomay, established under the "Home Help" scheme for the provision of social services in the home environment. The project envisages services to be provided within 12 months and to cover at least 84 users. This model will structure and standardize the provision of health-social patronage based on the specific training program developed by the Ministry of Health for medical specialists and social services specialists who will provide patronage care. The program / methodology developed by the Ministry of Health for the provision of patronage care will create unified guidelines for the specialists providing services on the territory of the municipality. The envisaged project activities are: - Provision of hourly mobile integrated health and social services for disabled and elderly people unable to self-handle; - Providing rehabilitation and psychological support; - Training and supervision of staff; - Material and technical support for mobile work. The implementation of these activities will contribute to providing a new type of integrated health care and social care services for long-term care in the community that will be of the necessary quality and accessibility and will help the process of deinstitutionalization of care for the elderly and the elderly.
