This project proposal is in full compliance with the Action Plan for the period 2018-2021 for the implementation of the National Long-Term Care Strategy. The project is planned to cover at least 50 elderly people and people with permanent disabilities living in the municipality of Yakimovo. The project envisages provision of hourly mobile integrated health and social services that ensure continuity of care through the participation of various specialists and stakeholders , confirming the practice of providing integrated care through the provision of cross-sectoral services. The implementation of the activities will allow for better individualisation of the support offered, a more accurate assessment of the needs and the implementation of a multidisciplinary approach.
