The project proposal aims to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of services in the home environment and capacity building for their provision on the territory of Aksakovo Municipality. The project will provide hourly integrated health and social services for people with disabilities and the elderly who are unable to self-service in the territory of Aksakovo municipality by health professionals and social workers. Within the framework of the project proposal, a model for providing patronage care will be set up, a mobile service delivery base will be created, staff capacity will be built for effective service delivery through inclusion in training to implement the Patronage Care Methodology.
