Providing patronage care for elderly people and people with disabilities in Plovdiv Municipality

This project corresponds to the Action Plan for the period 2018-2021 for the implementation of the National Long-Term Care Strategy and is aimed at creating an integrated medico-social service for elderly people and persons with disability, incl. with severe chronic diseases and permanent disabilities - "Patronage Care" - on the territory of the city of Plovdiv. The integrated mobile service "Patronage Care" will provide consumers with the opportunity to benefit from medico-social services in their homes, according to their individual needs. For this purpose, 53 specialists (doctors, medical staff, psychologists, social workers, service management team) will be appointed within the project to care for 561 persons from the target group. A dedicated office for service staff will also be set up to provide users with a permanent and direct connection with the mobile teams who provide services to the users, thus facilitating their access to services and their timely provision. It is planned for the integrated mobile medico-social service "Patronage Care" to be provided for a period of 12 months. Within the timeframe of the project it will be free for the users.
