Patronage of elderly people and people with disabilities in Lukovit Municipality

The project proposal "Patronage Care for the Elderly and Disabled People in Lukovit Municipality" will provide assistance and support to at least 60 people with disabilities and elderly people over 65 who are living on the territory of the municipality and are not able of self-care. In the frame of the project a new adult patronage model will be introduced and implemented providing a comprehensive health and social care services in a home environment for deprived persons up to 2 hours a day, depending on their needs. The project is also targeted to clients in a recovery period after hospital treatment who have an individual assessment. The hourly integrated health and social services, incl. psychological help and support that are the basis of the new model of patronage care will include home care services, medical care, rehabilitation and psychological support. The services will be mobile and will be provided following an agreed schedule. For this purpose, a suitable motor vehicle will be purchased, necessary for the timely transport of the service team, and will allow the provision of the patronage care in a home environment in all the settlements of the municipality, of the most distant municipal centre. The team for providing the new type of service includes: home assistants, nurses, rehabilitator and psychologist as well as dispatcher and driver. All the members of the team will undergo adequate training to apply the new type of care. The project budget will cover the purchase of fuel necessary for the transport of the team, as well as materials and consumables for the medical care. The project duration is 21 months, the hourly mobile integrated services will be provided for a period of 12 months.
