"Patronage care provision for elderly and disabled people in Gabrovo municipality|"

The present project proposal has a goal to improve the quality of life and to expand the social inclusion opportunities of the elderly and disabled people in Gabrovo municipality through provision of patronage home care. Specific goals: - to improve the quality of life through provision of specific social-health services, relevant to the needs; - to create a model of provision of patronage care for elderly and disabled people; - to build a capacity of quality patronage care provision. The project target groups are: - elderly people above 65 years with limitations or unable to care of themselves; - people with disabilities and their families; - employees of organizations and institutions, involved with the process of deinstitutionalization of elderly/employees of social and health services providers The following main activities will be realized: 1. Creation of conditions for “Patronage care” service provision; 2. Regulation and providing for the provision of the actual provision of “Patronage care” service 3. “Patronage care” social-health service provision. The project realization will last 18 months, staring with activities, related to creating of condition for the service provision,, including personal training, regulating the service for provision in Gabrovo municipality, selection of external contractors etc. The provision of “Patronage care” social-health service will be realized for a period of 12 months and will answer the needs of 224 users.
