The implementation of the project proposal will provide patronage care by providing integrated medical and social services to elderly people and persons with disabilities in Beloslav municipality. The provision of health-social services in the everyday life of 50 disabled persons and people over 65 years old with limited or inability to self-service will help to overcome social exclusion and poverty. The implementation of the project proposal will complement and build on the activity of the unit established under the project "Home help" financed under the HRD OP for 2007-2013. to the existing home social patronage and will continue to provide integrated part-time services provided under the Independent Living project funded under the HRD OP 2014-2020. Depending on the individual needs of the needy, hourly medical and social services will be provided after individual plans have been developed and delivery schedules, through the implementation of the project proposal will be provided health-social services, combining complex actions in the direction of providing long-term care at home environment.
