Patronage of elderly people and disabled people in Valchi Dol municipality

The project will be implemented within 14 months and will be managed by the Municipality of Valchi Dol through the Directorate "Investment Policy and Humanitarian Activities".A model of patronage care for the elderly and disabled will be built to provide hourly mobile integrated health and social services in their homes.A team will be set up for the project and will include medical professionals, psychologists and social workers who will provide health and social services.The team will also include persons who do not have a professional qualification in the sphere of health care or social activities but who will undergo training under a program developed by the Ministry of Health for provision of non-specialized services for the persons of the target group (cleaning, etc.).Mobile hourly integrated social-health services aim to support the daily lives of people in the target group so that they can stay in their family environment.The inability to self-handle and / or limitations will be established through a social assessment of the person's needs.The services that one person receives will be based on the person's individual needs and will not exceed 2 hours a day.The project will include 50 persons who will use the service (the target group), as well as 11 employees, who will provide the patronage care, namely: 1 pc. specialists; 1 piece. social workers; 1 pc psychologists; 7 pcs. associate social activities.The following integrated services will be provided on the territory of the Municipality of Valchi Dol: injection, change of bandages, measurement of blood sugar, etc., according to the needs of the target group eligible to be provided outside of the medical facility; psychological support and counseling; social work and attendance to various institutions, help in the household by a collaborator social activities, etc.
