Patronage care in Belene Municipality

In the last few years the number of people who in result of different reasons as health issues, poverty, lack of income, increases. They need particular social and health care. Our project proposal provides a long-term care for persons with chronic illnesses, permanent disabilities or elderly people. The model for integrated health and social services in home environment will provide medical and social support from medical professionals and home assistants. The service will be provided based on an individual assessment of each person. With the implementation of the project, elderly people are expected to receive the care they need, living in their own environment. These people are mostly with chronic illness and permanent disabilities who are left alone in their homes. Homebased cares, provided within the project proposal are more than mechanical assembly of medical cares and social services. They are whole new approach which allows despite all illnesses and misfortunes a man to grow old in his own home, where are all his memories of his entire life. The activities cover entirely elderly people over 65 years old, people with limited or inability to self-service, as well as to people with permanent disabilities and their families, people who have no close relatives and have a social need for hourly services. It will be provided a complex of health and social services and support in home environment for 50 people. The mobile care service includes activities aimed at improving the quality of life of the elderlies and these with disabilities, depending on the specific condition and limitations. Through the implementation of the activities, the quality of life and the opportunities for social inclusion of disabled people will be improved byproviding a network of home based services and creating suitable ( material and staff ) capacity to provide them.
