Patronage cares for adults and persons with disabilities

The project is designed to provide better quality of life to adults, who cannot satisfy their own needs by themselves and are in risk of poverty and social exclusion and people with disabilities . The project provides access to key mobile integrated health-social services for leading an independent and full-filing life to more than 60 citizens of municipality of Chernoochene. The development of home-based and community-based social services is the basic condition to insure equal and efficient access to key services for the citizens of municipality of Chernoochene, which are in need of long-term care. The project will provide conditions for independent and dignified life for 60 elderly and disabled people by improving their access to and the quality of integrated health-social services, expanding their network across the municipality of Chernoochene, fostering integrated services provision. This will contribute to providing comprehensive support to 60 families currently giving care to disabled persons, disabled children and elderly. The overall objective of the project is improving the quality of life and opportunities for social inclusion of people with disabilities and adults through ensuring of number of services in home environment and establishing of adequate human resources for their delivery. The specific objective of the project is to establish a model of patronage cares for adults and persons with disabilities, including chronical diseases and permanent disabilities with the aim of ensuring of mobile integrated health-social services at their homes.
