Support at home

The execution of this project proposal is to establish a sustainable model to offer integrated health and social services for people with deficiencies and their families as well as elderly people above 65 years old that are unable to take care of themselves and that are at risk of social isolation.Within the framework of the project hourly-based services will be offered, alone or in conjunction, including healthcare and activities of social support in a home environment.A team will be formed to oversee the project and they will be accountable for the timely execution and the quality of services offered.The team will begin its mandate with an information campaign outlining the purpose and scope of the project and establishing the conditions of use for the services.A team will be appointed to guide the activities of the integrated services offered, who will organise and coordinate the ensemble of activities within the individual needs and targeted groups.Personnel will be hired, registered nurses, to offer healthcare and medical oversight depending of the needs of the person, following doctor established treatments or guidelines,etc.; medical orderlies to help with the following activities: help the users with their personal and home hygiene, supplying the help needed when preparing food, groceries, assistance in obtaining administrative services, etc.Training will be provided to the personnel offering the services in accordance with a developed M3 program, as well as supervision.Within an established need of the users there will be an offered opportunity for rehabilitative services.Overcoming loneliness, social isolation as well as guided support to lead an independent lifestyle, if there is a need psychological services can also be offered.For the objectives of the project construction and installation activities will take place over a place that is property of the municipality, defined as an office for the management team of the service and adjacent bathrooms
