This project proposal is in full compliance with the Action Plan for the period 2018-2021 for the implementation of the National Long-Term Care Strategy. The project is planned to cover at least 50 elderly people and people with permanent disabilities, creating a partnership between the municipalities of Rila and Kocherinovo. The project includes provision of hourly mobile integrated health and social services, staff training and supervision as well as the purchase of transport means to ensure mobility in the course of the activity. As a result of the implementation of the activity, it is expected to ensure the continuity of the care through the participation of different specialists and stakeholders, strengthening the practice of providing complex care through intersectoral services which allow for better individualisation of the offered support, a more accurate assessment of the needs and applying a multidisciplinary approach.
