Through realization of the activities in this project medical specialists and specialists in the field of social services will provide health care and medical monitoring and also will realize social work in support of people with disabilities, chronic ilnessess and elderly people who need help in the performance of daily activities in the municipalities of Loznitsa and Samuil. The integrated health and social services will be provided to at least 65 persons by the target group for 12 months. There will be revealed a total of 14 jobs, 7 of which in Loznitsa municipality and 7 in Samuil municipality, including 4 medical specialists and six home sanitariums, two driver and two dispatcher, which will be distributed proportionally in the two municipalities. For the employed workers will be conducted introducing education as well as ongoing supervision. There are planned repairs, furnishings and equipment of appropriate premises, necessary to provide the activities of specialist’s in Samuil municipality and purchasing of a vehicle for the specialist’s mobile work in Loznitsa municipality. The project directly corresponds to the objectives and priorities of the Priority axis 2 „Reducing poverty and promoting social inclusion”; Investment priority №3 „Improving access to services which are with accessible prices, resistant and high-quality, including health and social services of general interest”; Specific aim №2 of IP 3 “Reducing the number of elderly people and people with disabilities which are accommodated in institutions through provision of social and health services in the community, including services for long-term care”.
