The general objective of the project is to improve the quality of life of elderly people and people with disabilities from the region of the Municipality of Smolyan. Specific objectives: 1. Support to the disabled people by providing mobile integrated health-social services in their homes, according to their needs. 2. Enlargement the capacity, variety and purposefulness of social services in the community and improvement the quality of life of elderly and disabled people and their families. For the realization of the project will be implemented the following activities: Purchase of motor vehicle and supply of equipment, that is necessary about the quality providing of the services and the project implementation; Selection of users and staff; Staff training and supervision; Providing integrated health and social services. The services will be provided by a team, including coordinator/dispatcher, driver, nurse, rehabilitator, psychologist and 35 home assistants. The hourly mobile integrated health-social services will be implemented by a Unit/Department if the Municipal administration Smolyan, which will be assigned the patronage care as a service of general economic interest and in accordance with program / methodology / developed by the Ministry of Health about patronage care and quality of service standards. 120 elderly people and people with disabilities from the region of the Municipality of Smolyan will benefit from the services. Every person from the target group will use health and social services for 2 hours per day. The services may be the following: - injections, changing dressings, blood glucose measurement, rehabilitation, etc., according to the needs of the users who are eligible to be provided outside of a medical facility; - psychological support and consultation; - escorting/accompaniment to different institutions; - household help. The total cost of the project is 318 192 lv./BGN and the duration of its implementation is 16 months.
