Project “Patronage care in Lom Municipality” aims at improving the quality of life and the opportunities for social inclusion of disabled people and elderly people on the territory of Lom Municipality by providing a net of services in home environment and creating conditions for their quality implementation /of the services/. Within 16 months the following activities will be implemented under the project: 1. Providing hourly mobile integrated health-social services for needed people with disabilities and elderly people in Lom Municipality. 2. Education and supervision of the staff 3. Equipping proper premises, including buying of a vehicle for doing a mobile work 4. Project organization and management activities 5. Information and publicity activities The project realization will improve the quality of life and the opportunities for social inclusion of 91 elderly people and people with disabilities, including ones with chronic diseases and permanent damages. The successful realization of the project activities will create real preconditions for going back on the labour market of people who take care for their relatives with disabilities and elderly people on the territory of Lom Municipality. The project realization will create a new model for providing hourly mobile integrated health-social services in home environment.
