"Patronage care for the elderly people and the persons with disabilities in the municipality of Gotse Delchev"

The main objective of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in the municipality of Gotse Delchev by providing hourly, mobile, integrated health and social services in a home environment. The specific objectives are: - Expanding the scope of services provided in the municipality of Gotse Delchev in a home environment for people with disabilities and elderly ones, over 65 years old by introducing and applying a patronage care model. - Building local level capacity (facilities and staff related) for providing a network of quality and affordable, integrated health and social services for disabled and elderly people. The achievement of the project objectives will be ensured through the following activities: provision of hourly mobile integrated health and social services for disabled and elderly people in need; providing psychological support and/ or consulting to people of the target group; training and supervision of the staff; furnishing and equipping appropriate premises, including the purchase of a vehicle for carrying out mobile work. A total of 53 persons, residents of Gotse Delchev Municipality, will be included in the project, under the conditions of non-discrimination based on sex, disability, age, ethnicity etc., who are representatives of the following target groups: people with disabilities and their families; elderly people over 65 with limited abilities or inability to self-handle. Within 12 months, the target group will be provided with hourly mobile integrated health and social services tailored to the individual needs of each user. A basic prerequisite for the quality of the offered services will be the availability of the facilities provided for the project (mobile equipment and furniture for medical and rehabilitation services, transport means, etc.) and a team for management and provision of services (qualified specialists, trained and motivated staff).
